             Checks payable to the           KalihiYMCA

$55 per month

The following months are combined:

Dec & Jan | Mar &. Apr |  May & June

Payment is due the FIRST SCHOOL DAY of each month.

Failure to pay within 5 school days from the beginning of the month may result in termination from the program.
